Purpose

The purpose of this notice is to explain how Helen Hodder Physiotherapy collects and uses your personal information and how we comply with data protection law.

The Personal Data We Process and What We Do With It

We record and use the following categories of personal data: name, address, telephone numbers, email address, date of birth, health information medical history, diagnosis and treatment data. Our lawful basis of processing this data is one of contract and, for the health information, the provision of health-related services as a physiotherapy clinic.

If you are receiving physiotherapy treatment, Helen Hodder may need to share personal data with other medical professionals involved in your treatment e.g. your GP or specialist consultant. Helen Hodder Physiotherapy will obtain your consent at the time to share your personal data with these parties. Helen Hodder may need to share your personal data with your health insurer to facilitate reimbursement of fees on your behalf and, on the basis that you provide your health insurance details to us, this will be deemed to be your consent that we may share applicable personal data with them.

Retaining Your Personal Data

Whilst you are receiving treatment arranged with, or attending sessions/workshops put on by Helen Hodder Physiotherapy, she will continue to store and use your personal data. If you have been receiving physiotherapy, once you have been discharged, Helen Hodder will be required to retain your personal data for a minimum of 8 years.

Your Rights

As Helen Hodder Physiotherapy processes your personal data, you have certain rights. These are a right of access, a right of rectification, a right of erasure and a right to restrict processing.

You may request a copy of your data at any time. Please make such a request in writing or by email using the contact details shown below. Please provide the following information: your name, address, telephone number, email address and details of the information you require. Helen Hodder Physiotherapy will need to verify your identity so we may ask for a copy of your passport, driving license and/or recent utility bill.

If you believe any of the personal data that Helen Hodder holds on you is inaccurate or incomplete; or you believe she should erase your data; or you wish Helen Hodder Physiotherapy to stop storing or using your data, please contact Helen directly using the contact details below and any necessary changes to your data will be made promptly.

Data Breaches

Should your personal data that Helen Hodder Physiotherapy controls be lost, stolen or otherwise breached, where this constitutes a high risk to your rights and freedoms, you will be contacted without delay. We will give you the contact details of the responsible person dealing with the breach, explain to you the nature of the breach and the steps being taken to deal with it.
